Fail upgrade from +9 to +10 with 100% success rate 30x server

Hello, today i've been mixed one chaos nature bow in the website services - craft item system, and while trying to increase it to +10 the goblin show 100% success rate and even with that the bow combination was failed. Waiting for gm's reply . Thank you !

In details about lost items: EXC chaos nature bow + 9 + 12opt + luck + inc dmg 20/lvl (as seen in the image bellow ) , 1 bless , 1 soul, 1 chaos and around 41kk fee for chaos machine creation

Was the Safety Mode is activated? : I have no ideea what's that ?
